Together Clinic, LLC is a healthcare technology start-up company with software aimed at bridging the gap between patients and providers in the management of chronic disease. Other solutions are focusing on only 30-day readmission rates, can be very expensive for health care systems to implement and have little incentive for patient compliance. Together Clinic is creating a mutually beneficial patient and provider community that will decrease patient readmissions, promote patient satisfaction, and decrease health system costs.
